Former user doh... I was trying to get it done quickly with my kids running around. My mistake.
S.M.A.R.T. Error is set to IMMEDIATELY

you've posted screenshot from Alert Services. We need Alert Settings

Thanks for taking another look at this Former user.
When I go to System -> Alert Settings I don't see a "S.M.A.R.T. error". As below you'll see I have "E-Mail" and "SNMP Trap".
There is an "Add" button but I don't see any obvious way to add a "S.M.A.R.T. error" Alert Service.

actually, I've somehow missed this and never addressed this issue:

This should not be possible. What option do you have selected for "S.M.A.R.T. error" in System -> Alert Settings?
I am unable to get the S.M.A.R.T. Service to send me an email when a HDD is over the Critical temperature limit.
Firstly I am able to successfully send/receive a test email from System / Email
and I am able to successfully send/receive a test email from System / Alert Services / Email
My drives are running about 32 degrees Celsius
However even when I setup the SMART Service to a Critical Temp of 10 degrees C, and a check interval of 1 minute, I get no emails from FreeNAS alerting me of the over-temp situation.
After rebooting my FreeNAS server and logging back in I noticed that I had some new alerts related to HDD temps (but still no emails). So for starters it looks like the settings are taking until the system is rebooted. I think this is related to . Just in case the reboot helped, I went back into the SMART Service, set a critical temp to 40, waited a couple mins, and then set it back to 10. Still no email (and also no new alerts).
After the reboot, however, the console displayed alerts every minute (consistent with the 1 minute interval I setup). STill no emails though. Changing the interval (to say 30 mins) or changing the critical temp (to say 40) has no effect on the console, it continues to alert every 1 minute with a critical limit of 10.
