SCALE 20.12 Web Interface Ticket Creation doesn't upload additional files

Description

Testing ticket creation from within TrueNAS-SCALE-20.12-MASTER-20201214-052920 (SCALE 20.12 candidate) web interface. I filled the fields, set Attach Debug, and chose to upload a screenshot with the ticket. The ticket was created and the debug file appeared, but the screenshot was not attached to the ticket. (see related ticket).

I tried the process again, this time without adding a debug file. The result was the same: the ticket was created but the screenshot was not added correctly.
