This is still not resolved via 3-12-21 nightly. Exact issue as reported.

Can you please provide a debug (navigate to System -> Advanced, click save debug) and upload the file to the Attachments:Private area in this ticket?
Files uploaded into the private area in Attachments are only accessible by the developers.

The "Driver" field is always reported to be required even if filled in (pic attached):
This field is required
If the field is empty it will give two errors:
Driver is required.
This field is required
This seems similar to https://ixsystems.atlassian.net/browse/NAS-106410#icft=NAS-106410 but was never looked to be fixed.
