When I generate a CSR, the SAN names are correct:
However after signing it with the CA certificate, the SAN gets mangled:
So now that I've added the CA certificate as trusted on my client, the certificates are trusted but can never match the domain names they're being used for because they have an extra DNS: prefix on them. Example view from firefox:
